An application form for the legendary Berkeley Poetry Conference, listing two weeks of seminars and readings by Snyder, Ginsberg, Duncan, Olson, Creeley and others. Folded for mailing else fine. Quite rare; a broadside announcement shown with two holdings on WorldCat (Yale and Buffalo), another mimeo sheet presumed to have been handed out to seminar participants for sale (for $518) online; this would likely have preceded those.
